Adverse events

Investigators are required to immediately report any adverse events relating to a project to the Office of Research Ethics and Integrity (OREI) and Senior Biosafety Officer (SBO), including those that have occurred at non-QUT facilities or sites. Supervisors and Facility Managers should also be immediately notified of any incident.

Examples include:

  • finding a window broken
  • break and enter
  • samples missing
  • exposure to aerosols

To report an incident: call the Senior Biosafety Officer (3138 7411) or the Research Ethics Coordinator (3138 2091).

Researchers should consider whether to suspend or modify the project if the risks to people and the environment have changed and immediately advise the Research Ethics Coordinator of this action.

The UBC may report directly to the OGTR or Department of Agriculture in respect of any accident or incident involving GMOs or quarantine materials which has resulted in an unintentional release and/or non-compliance with the regulations.
